A 34 year old male with pan brachial plexus injury under went brachial plexus exploration .
ICN 345 to musculocutaneous nerve transfer was done.
Spinal accessory to  supraspinatus nerve transfer.
Phrenic nerve to axillary transfer via nerve graft.
Pt showing good elbow flexion with physiotherapy after one year.

this is a post operative results of patient who suffered erbs palsy during child birth. As the plexus recovered  patient developed  shoulder contracture which resulted in inability to raise arm. Subscapularis muscle contracture results in internal rotation , adducted arm . It results in inability to raise arm over shoulder , its results in bungle post deformity. Surgery was done to release subscapularis muscle , pectoralis major muscle was partial release was done by cutting on muscle fascia. LD and teres major were transferred to improve external rotation . Pan brachial plexus injury which was operated. All trunks were found to be avulsed and nerve transfer were done . Spinal accessory nerve was transferred to suprascapular nerve to innervate suprascapular muscle for shoulder abduction. Successful innervation results in 40 degrees of shoulder abduction which is useful in day to day activities.
